Of the new cases confirmed on Monday, 2,834 were in the central region of Mazowieckie, which includes the national capital Warsaw.
Meanwhile, 2,694 new infections were reported in the southern coal mining region of Silesia.
The southwestern province of Dolnośląskie, which is home to the major city of Wrocław, had the third-highest number of new infections confirmed by officials on Monday, at 1,534.
The latest deaths in Poland’s coronavirus outbreak are 37 people with pre-existing medical conditions and 11 who died directly because of COVID-19, the Polish health ministry said.
On Sunday, Poland confirmed 131 deaths and 29,253 new coronavirus infections nationwide, compared with 448 deaths and 31,757 fresh cases a day earlier.
On Friday, March 26, the Polish health ministry confirmed 35,143 new single-day cases, the most since the pandemic hit the country last March.
On November 25, the country reported its highest daily toll of 674 deaths related to the coronavirus.
Poland's first case of coronavirus infection was reported on March 4, 2020.
29,920 in hospitals, 423,889 quarantined
The health ministry announced on Monday morning that 29,920 COVID-19 patients were in hospitals nationwide, 2,985 of them on ventilators, with a further 423,889 people quarantined for possible coronavirus exposure.
Meanwhile, 1,824,431 people have now recovered from COVID-19 throughout the country, including 25,509 over the last 24 hours, the health ministry also said.
