Of the new cases confirmed on Monday, 1,171 were in the densely populated southern coal-mining region of Silesia.
Meanwhile, 1,100 new infections were reported in the central region of Mazowieckie, which includes the national capital Warsaw.
The southwestern province of Dolnośląskie, which is home to the major city of Wrocław, had the third-highest number of new infections confirmed by officials on Monday, at 747.
The latest deaths in Poland’s coronavirus outbreak are 53 people with pre-existing medical conditions and 48 who died directly because of COVID-19, the Polish health ministry said.
On Sunday, Poland confirmed 207 deaths and 12,153 new coronavirus infections nationwide, compared with 616 deaths and 15,763 fresh cases a day earlier.
On April 8, the country reported its highest daily toll of 954 deaths related to the coronavirus.
On April 1, the Polish health ministry confirmed 35,251 new single-day cases, the most since the pandemic hit the country more than a year ago.
Poland's first case of coronavirus infection was reported on March 4, 2020.
31,612 in hospitals, 265,383 quarantined
The health ministry announced on Monday morning that 31,612 COVID-19 patients were in hospitals nationwide, 3,346 of them on ventilators, with a further 265,383 people quarantined for possible coronavirus exposure.
Meanwhile, 2,334,978 people have now recovered from COVID-19 throughout the country, including 23,833 over the last 24 hours, the health ministry also said.
