Poland advanced to the competition's last four after defeating Spain 2-0 and the Czech Republic 2-1.
Italy, meanwhile, had a bye in the tournament's round of 16, and secured a 2-1 victory over Japan in the quarterfinals.
One of the highlights of Monday's semifinal clash in Malaga, Spain will be Poland's world No. 2 Iga Świątek going head-to-head with Italy's world No. 4 Jasmine Paolini.
In the quarterfinals, Świątek overcame the Czech Republic's Linda Nosková 7-6, 4-6, 7-5 in the singles and also teamed up with countrywoman Katarzyna Kawa to win a decisive doubles match against the Czech pair of Marie Bouzková and Katerina Siniaková.
The Polish duo triumphed 6-2, 6-4, securing Poland’s place in a historic semifinal, public broadcaster Polish Radio’s IAR news agency reported.
"It's amazing to be making history for Poland; it's a dream come true," Świątek said.
