Hurkacz and Isner saw off the Netherlands' Wesley Koolhof and Britain's Neal Skupski 7-6, 6-4 in the championship match on Saturday to claim their first title together.
Hurkacz in September won the men's doubles title at the Moselle Open tournament in the French city of Metz, partnering with countryman Jan Zieliński.
In November 2020, Hurkacz won the men’s doubles at the Paris Masters together with Canada’s Félix Auger-Aliassime.
That win marked the first ATP doubles title for the Polish player.
