Heavyweights Legia Warsaw finished third in the Polish premier league, four points behind Jagiellonia and Śląsk.
Defending champions Raków Częstochowa ended their top-flight campaign in seventh place.
In the final round of matches, Jagiellonia posted a comfortable 3-0 win against Warta Poznań at home on Saturday, while Śląsk defeated Raków in the southern city of Częstochowa 2-1.
Legia, meanwhile, beat Zagłębie Lubin 2-1 in front of a home crowd in the Polish capital.
Jagiellonia, Śląsk and Legia will represent Poland in European competitions, which are due to begin later this year. Joining them will be second-tier outfit Wisła Kraków, who won the Polish Cup in early May.
Jagiellonia will represent Poland in the Champions League next season; Wisła will have a chance to play in the Europa League, while Śląsk and Legia will both compete in the Europa Conference League.
Jagiellonia and Wisła will face off against each other for the national Super Cup title later this year in a curtain-raiser to the next football season.
