Fresh infections were more than 3,500 up on last Sunday.
Officials have warned that the number of Poles in hospital with COVID-19 is on the rise, as is the number of patients on ventilators, amid a surging third wave of the epidemic.
Of the new cases confirmed on Sunday, the most – 3,430 – were in the central region of Mazowieckie, which includes the national capital Warsaw.
Regional restrictions
On Monday, a partial lockdown comes into force in the central Mazowieckie region and the western province of Lubuskie.
Restrictions will be imposed on shopping malls there, while hotels will be closed, and cultural institutions, swimming pools and sports facilities will be shut.
A partial lockdown came into force in Poland's northern Pomerania province on Saturday.
Meanwhile, a range of restrictions on public life were last month introduced in the Warmińsko-Mazurskie region of northeastern Poland, another province in which the level of infections has caused concern.

Pandemic restrictions in the rest of the country will remain unchanged until at least March 28 following an easing of containment measures earlier this year.
On November 25, the country reported its highest daily toll of 674 deaths related to the coronavirus.
On November 7, the Polish health ministry confirmed 27,875 new single-day cases, the most since the pandemic hit the country last March.
Poland's first case of coronavirus infection was reported on March 4, 2020.
More than 4.5 million COVID-19 vaccine shots have been administered in Poland so far, officials announced on Sunday. Over 1.6 million people have received a second shot.
