The Polish ace is in the running with 30 other players, including previous winners Cristiano Ronaldo and Lionel Messi, who have both won five awards each.
The nomination comes after Lewandowski this year alone scored a whopping 39 goals in 42 games, including 34 for his Bayern Munich side and five in UEFA Euro 2020 qualifiers for Poland.
The winner of the 2019 Ballon d’Or award will be announced on December 2.
